Greenleaf can support you through your entire NDIS journey.  We have a number of different programs to fulfil the needs of the people with disabilities in the region.

Supported Independent Living (SIL)

Supported Independent Living through our Greenleaf Accommodation services division is one of the core services that Greenleaf Care Services provides. In fact, we are highly respected and well known for our SIL services. Since our establishment in 2016, we have supported a large number of NDIS participants through these accommodation services. Greenleaf Care Services currently has a number of houses around Canberra, as well as in Goulburn and the Gold Coast and we support a broad range of participants, including those with psychosocial disabilities, intellectual disabilities and coexisting conditions. More houses are being added to our services across existing locations and new locations. Greenleaf Care Services provides a broad range of accommodation services that assist participants to: develop individual program plans that outline goals, aspirations and desires for independent living, develop and maintain relationships and community connections, and access social and community activities. We also assist with managing and attending appointments, and providing assistance with any other administrative tasks needed to support and maintain independent living.

Respite (STA) & Medium Term Accommodation (MTA)

Short Term Accommodation services, or respite services as they are commonly known, are planned short term, time limited breaks for NDIS participants to have time away from the family home. It assists the ongoing relationships of family members and other unpaid carers of NDIS participants – with the intention that families and/or carers resume care at the end of the respite period. Short Term Accommodation services are provided in a 'respite' house where the NDIS participant stays overnight or longer. The benefits of Short Term Accommodation for participants include that they can use the opportunity to take a break from their regular schedules, connect and socialise with new people, and experience a supported shared living environment. Respite can also be an opportunity for carers to get a well-deserved break so that they’re refreshed and reenergised when reunited with their client.

Support Coordination

Once a participant's NDIS plan is approved, Greenleaf Care Service's support coordination program then assists the participant to implement the plan to achieve their unique and desired goals. Greenleaf Care Service's support coordination works closely with participants to ensure a combination of supports are used to increase their capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in their community. Support coordination can also include initial assistance with linking participants with the right providers to meet their needs, offering support in sourcing providers, developing participant resilience in their own network, community coordination for a range of funded and mainstream support services, and building on informal supports.

Social, Community and Home Care

This service offers NDIS participants support and assistance to engage with their community. This can include social and recreational activities, either individually or in a group. Greenleaf Care Service's social, community and home care service also incorporates additional support to assist participants to attend centre-based programs to develop skills to help them access and engage with the community independently. These services also assist NDIS participants with personal care and domestic assistance, such as shopping, cooking, cleaning, recreation and transportation.

Case Management

If a person with disability would like to join the NDIS, they will need to meet the program's Access Requirements. As part of Greenleaf Care Service’s corporate social responsibility program, we provide assistance to undertake and complete the application process and gather together information and evidence to position the application in the best possible light for approval. This service is entirely complimentary and does not place any obligation on anyone to engage any further with Greenleaf Care Services. At Greenleaf Care Services, we believe everyone deserves the support to live an independent life which is why we strive to give everyone the same opportunity to receive NDIS support.

Plan Management

Plan management is a Financial Intermediary Service for NDIS Participants. As a registered NDIS Plan Manager, Greenleaf Care Services will manage the financial and administrative tasks of participants' NDIS Plan. Plan Management will give more choice, control and flexibility to participants on their plan. Participants can choose their providers, even they are not registered with NDIS, Greenleaf Plan Manager will pay the invoices and keep all records according to NDIS requirements.
